Quality Assurance Check for Digital Documents

This form is for individuals who have been trained in Digital Accessibility Remediation to submit their documents for a final check and approval from the Digital Accessibility Team.

  • Checks and pass/fail results will be communicated to the submitter within 3 business days from submission PER document.

  • If you require an accompanying Compliance Verification Form, please select an option below. If your client has a specific form they would like to be included with the final document, please add a link in the comment section or attach the form to this ticket.

  • If you would like your document to be finalized as a PDF fillable form, please submit either a Standard Ticket or an Expedited Ticket.

  • If you need an expedited check, please contact the 508 Manager, Michaela Rizzo (mrizzo@ahpnet.com) when you submit this form.
