Please include here the document (if any)

April 2025 NOTICE:
Due to a reduction in remediation staff, remediation will be delayed past the standard 7-10 day turnaround. If you have a document that requires a quick turnaround, please submit an Expedited Doc Accessibility Remediation Request and contact the Digital Accessibility Manager (Michaela Rizzo mrizzo@ahpnet.com) directly to discuss expedition and 3rd-party referral options.

Thank you for your patience as we continue to build our AHP Accessibility program to better serve you! If you have any questions, suggestions or comments, please reach out to Michaela Rizzo (mrizzo@ahpnet.com).

The purpose of this form is to notify the Digital Accessibility Manager as early as possible about upcoming documents or website reviews that are high-level and/or have a tight turnaround timeline. The earlier the Digital Accessibility team is notified about these kinds of documents, the better we can resource plan and guarantee a date of delivery.

In general, the Digital Accessibility team nor our 3rd party vendors can complete an expedited request in 24 hours or less.

Please enter you project's full Deltek code, including subtasks.

Person making request

Document to be reviewed (if applicable)

Please use the attachment field at the top of this request to upload a copy of the document to be reviewed. If possible, please upload the original file format (Word/PowerPoint/Excel), as this significantly reduces remediation time.

You do not need to upload a document to request an expedited remediation turnaround, but we do encourage you to submit any draft or outline of the document in this form here. The Digital Accessibility team cannot guarantee a turnaround time if we do not have the final document, or if the final document delivered to the team is different in length/intensity of content than the draft first submitted in this ticket.

Please select an option below
Please select what you would like the 508 team to work on
*Once content is added to a template, it must be submitted to 508 again for a final PDF remediation (ex. Project creates a 508 webinar PPT template to send to external presenters for content. Once the presenter fills in the content, it will have to go through 508 once more).
Select final remediated document file type

It is standard practice to only post PDFs, PDF Fillable Forms, and Excel templates on Government owned, public-facing websites to avoid impersonation or copyright issues. If you would like a fillable template style document, please select “Fillable Form” above, and leave a comment with your desired final document type (Excel, Word, etc.).
Please select an option below. If no Alternative Text is given, remediation will be delayed.
The 508 team are not project content experts, and cannot glean relevant contextual information from an image or conduct data analyses of charts/graphs. For guidance, here are some quick tips regarding Alt Text from Harvard. Alternative Text is not necessary for decorative or filler images. If you have any questions about Alternative Text, please contact the 508 Coordinator.

If you submit a document for review, we will contact you within 24 business hours to confirm your request and provide a timeline for delivery.
